A lot of claims fail the first time
A survey of more than 500 US hospitals found that nearly 15% of claims were denied on first submission. More than half of those were eventually paid after appeal. In other words, much of that denial work was avoidable: the hospital did the treatment right, but the paperwork didn’t show it
Insurance claims and first-time rejection (US)
Why claims get rejected
Missing patient or policy details. Wrong or incomplete codes. Treatment that doesn’t match the pre-approval. Clinical notes that aren’t attached. Most of these come back to one thing: information being typed by hand into a claim, separately from the care record
Getting it right the first time
As insurance moves to electronic claims, including national platforms like NPHIES in Saudi Arabia, the fix is to build the claim from the same record the doctors and nurses use. Check it for missing information before it’s sent. Track pre-approvals so treatment and authorisation match. And look at your rejection reasons every month, so the same mistake doesn’t happen twice.
