The riskiest moment in the stay
Discharge is where information has to move fastest, and where it most often slips. A 2024 study of older patients found medication mismatches in 35% at admission, 20% during transfers within the hospital, and 49% at discharge. That’s nearly half of patients going home with a medicine list that doesn’t add up

Why discharge is so hard
Several teams all need to finish their part at the same time: doctors, nurses, pharmacy, billing. The summary is often written at the last minute. And comparing hospital medicines with home medicines is still done by hand, under time pressure
What better looks like
Plan the discharge from the day of admission. Build the summary as the stay goes on, not at the end. Let the system compare hospital and home medicines automatically and flag differences. Have the bill ready before the family asks for it. Give clear instructions in the patient’s own language. And book the follow-up appointment before they walk out the door
